COUNSELING PRACTICUM / INTERNSHIP APPLICATION INSTRUCTIONS
Counselor Education students requesting practicum/internship
placements in Palm Beach County should identify their placement
school of choice and obtain the signatures of the school principal and the clinical
educator indicating their agreement to the proposed placement before
submitting the application the the Department of Counselor Education.
- The following must be included with your application:
- a copy of security clearance
for the appropriate county
- an original substance abuse screening for internship only (check
security clearance link to determine which counties require substance screening)
- two copies of your current transcripts
- Applications must be typed (from your computer) not handwritten.
- Remember to sign and date your application.
- You must provide two additional copies of your application that are
as professional in appearance as the original.
- Please be specific as to whether this application is for a practicum
experience or internship experience.
- This application must be signed by your advisor and returned to department
of Counselor Education by January 31 for a fall placement or September 15
for a spring placement.
