Internship in School Counseling must
complete the
Intern
Profile form at the beginning of
each term. Completed forms
should be emailed as an attachment
to
Dr.
Villares in order to receive
your Intern user ID and password.

Practicum and Internship students
with a user ID and password should
access the
Electronic Weekly Summary Log
on a weekly basis and enter your
Individual, Small Group, Large
Group, Consultation and Indirect
Guidance activities.

Once the activities has been entered
into the database, print the page
and meet with your site supervisor
to discuss your activities and sign
off on your form.

Bring your signed form to class and
give it to your university
supervisor.

and bring it your first day of
