How to submit these documents
  1. Click on the document to be completed. Allow the form to appear on the screen.
  2. Fill form as needed.
  3. Save form to a location on your computer that you will be able to access later.
  4. Once the form is completed. Email the form file as an attachment to the assigned FIAT Area Coordinator.

Participation Logs
Please print out a participation log to complete with each visit to your assigned school. You will submit this completed form to the Good FIT coordinator at the conclusion of your internship.
